任何機構如欲申請駐場急救醫療服務,請於活動舉辦 2 星期前透過以下方式盡早遞交申請:

1. 填妥並遞交網上表格;或

2. 填妥本表格並郵寄往長沙灣郵政局郵政信箱編號80530香港急救醫療服務、電郵至medicalfirstresponder@yahoo.com或傳真至 (852) 31574287

·非牟利申請可以下載「申請急救醫療服務」表格，並將填妥之表格：

To apply for on-site medical cover, please make an application at least 2 weeks prior to the date of the eventby the followings methods as early as possible：

Complete theonline application form, OR

by post 長沙灣郵政局信箱編號80530

by email medicalfirstresponder@yahoo.com.hk or

by fax (31574287).

注意事项

1. 我們只會就非牟利活動或公眾安全相關的駐場急救服務申請收取最多每個活動物資及運 輸費用。
Application for non-profit events or related to public health and safety will not be charged or will only be charged for maximum for logistics. In order to ensure fair usage of services.
2. 商業活動之急救服務申請會被收取合理費用,所得收益將全數用作維持本隊日常運作。
Application for business events will be charged for reasonable price. All profit will be used for routine operations and support other voluntary medical services
3. 香港急救醫療服務會於收到申請後 1個工作天內與申請人聯絡,並於 14 個工作天內回覆是否接納申請。我們會按閣下所提供的資料進行評估並安排有關服務,並會盡快以電話、WhatsApp、電郵或書面通知申請人申請之結果。
如活動詳情有任何更改,請盡早聯絡我們以作安排。
Hong Kong Medical First Responder will contact the applicant within 2 working days and confirm the
acceptance within 14 working days. We will evaluate and arrange the services according to the information
provided. We will notify the applicant for the result of the application by phone, WhatsApp, e-mail, or in writing as
soon as possible. If there are any changes to the event details, please contact us immediately to make further
arrangements.arrangement for on-site services and contact purposes, and will not be provided to third parties.
5. 如因惡劣天氣或極端情況導致活動有所更改或取消,請盡早聯絡急救醫療服務或當值主管。一般而言,當服務開始前兩小時內天文台懸掛八號或以上颱風訊號或黑色暴雨警告訊號時,駐場醫療服務將會取消。
If your event is cancelled or changed due to adverse weather or extreme conditions, please contact our Medical
Services Coordination Unit or duty in-charged immediately. Generally speaking, services will be cancelled when the typhoon signal No. 8 or above or black rainstorm warning signal is hoisted within two hours before the service starts.
6.遞交申請即代表閣下同意上述須知所載的條款,香港急救醫療服務保留最終決定權。
By submitting an application, you agree to the terms set out in the above notice. Hong Kong Medical First Responder Service reserves the right to make the final decision.

我們的急救醫療服務由本隊人員提供。

On-site first aid service are provided by voluntary medical personnel from Hong Kong Medical First Responder Volunteer Service. All duty members are volunteers.

我們會就非牟利活動或與公眾安全相關的駐場急救服務申請收取最多每個活動每日 HK$1600物資及運輸費用。

Application for non-profit events or related to public health and safety will only be charged for maximum HK$1600 per event per day for consumables and logistics.

商業活動之急救服務申請會被收取費用，所得收益將全數用作維持本隊日常運作及行動。

Application for business events will be charged for reasonable price. All profit will be used for routine operations and support other voluntary services

香港急救醫療服務於收到申請後 1 個工作天內與申請人聯絡，並於7個工作天內回覆是否接納申請。我們會按閣下所提供的資料進行評估並安排有關服務，並會盡快以電話WhatsApp電郵或書面通知申請人申請之結果如活動詳情有任何更改，請盡早聯絡我們以作安排。
